I have finally solved the issue of Last Guardian2 ～黄泉の封印～. I wrongly assumed LapDiv.exe is extracting the files correctly, but checked the code and LG2 uses different compression for some files. So there are no corrupted midi files at all.
Test driver (same as before), example XML (with song titles copied from music menu this time) and lg2ext.exe extractor for lg2music.lap (with source):
